How to Request a Proxy Card

1. Request a Proxy Card by email to secretary@cjai.ca (info@cjai.ca is also accepted), or by postal mail to CJAI, 5830 Front Rd, Stella ON K0H 2S0. Include your full name and postal address.
2. We will match your request to our membership list and send you a serialized Proxy card that can only be used by you. It is valid for this single meeting only.
3. Complete the Proxy card and return it so that it is received by Sunday, July 12, 2026 at 11:59 p.m.. You can return it by:
   • Email to secretary@cjai.ca — you must include clear photographs of both sides of the card.
   • Canada Post, courier, or hand delivery to:

     CJAI HQ

     5830 Front Road

     Stella, ON K0H 2S0

4. Station staff will collect and distribute all received proxy cards to the named proxies when they arrive at the meeting. Only the named proxies may use these cards. If your proxy is unable to attend, the card is void and your vote cannot be counted.

Choosing Your Proxy

If you feel uncertain about the issues, you are encouraged to select proxies who you believe will be willing to listen to arguments and decide in a manner similar to as if you were there yourself. If you are certain you know all the facts, then give the vote to someone who agrees with your opinion.

There has been a great deal of questionable information circulating during the past month. Both sides will be presented at the meeting. Ensure your proxy is willing to listen.
